AmeriHealth recently sent out letters and published a Partners in Health Update^SM article inviting certain providers who have received a waiver from the Substance Abuse and Mental Health Services Administration (SAMHSA) to provide medication-assisted therapy (MAT) in an office setting, to submit a request form for recognition in the AmeriHealth New Jersey and AmeriHealth Pennsylvania Find a Doctor online tools as a MAT provider. We want to thank those providers who have submitted their completed forms and waivers through the dedicated MAT provider email.

However, some submissions we received were not submitted to the dedicated email address, ahMATprovider@amerihealth.com, or were missing a copy of the SAMHSA waiver. When submitting your recognition request form, please make sure to do the following:

• fill out all necessary information on the AmeriHealth New Jersey or AmeriHealth Pennsylvania recognition request form;
• send the form and a copy of your waiver to ahMATprovider@amerihealth.com with the subject line “<Your name> - MAT certification.”

If the recognition request form is sent by any other method, is incomplete, and/or does not contain a copy of the SAMHSA waiver, we will not be able to process your request and you will not be identified in the Find a Doctor online tools as a MAT provider.
