Sometimes our members need extra support when they are dealing with an
acute illness, a chronic condition, or multiple issues such as, cancer,
diabetes, and asthma. Often they look to their treating providers for this
additional support.
Our dedicated registered nurse case managers can collaborate with you to
create a plan of care and support the medical treatment goals you have
established.
Case managers can:
- Collaborate with patients and their physicians to identify needs, develop
goals, and work towards resolution
- Assist in obtaining in-network medical equipment and care from
specialist
- Provide support to patients and their families during complex disease
processes
- Educate patients about the numerous benefits available as an AmeriHealth
New Jersey member
- Assist members in achieving an optimal level of health and wellness and
empower them to maintain this goal
- Facilitate management of high risk maternity members
