Introducing Commit2WellnessSM Rewards (NJ only)

May 31, 2012

AmeriHealth New Jersey recently introduced a new incentive program ? Commit2Wellness Rewards ? that offers commercial members an opportunity to earn Wellness Points for healthy behavior. The Commit2Wellness Rewards program replaces the AmeriHealth Healthy Lifestyles[]SM[] program for most AmeriHealth New Jersey members.* Through the Commit2Wellness Rewards program, eligible members receive Wellness Points upon completion of certain healthy activities, such as having an office visit with their primary care physician (PCP) or gynecologist, going to the gym, or attending two nutrition counseling sessions with a registered dietitian. To redeem Wellness Points, members must first complete an online Personal Health Profile and visit their PCP or gynecologist. Completion of these two activities alone will earn the member 50 Wellness Points that can be redeemed for debit or gift cards to a variety of retailers. Each Wellness Point is worth one dollar. Members can call []1-800-275-2583[] for more information on this new program. [

] *The Commit2Wellness Rewards program is not available to AmeriHealth New Jersey Medicare Advantage HMO members and certain groups. Members should contact Customer Service to verify their eligibility. [
