We would like to advise you of a change in the submission process for
Continuity of Care (COC) requests. There are now two separate forms for your
patients who have insurance through AmeriHealth: one for patients with an
AmeriHealth New Jersey health plan and one for patients with an AmeriHealth
plan in Pennsylvania or Delaware.
Please select the COC form that corresponds to the insurance plan of your
patient as these forms contain location-specific fax numbers and mailing
addresses. The updated COC forms can be found on the
Provider Forms page of the
AmeriHealth website.
