Health Coaches are available 24/7/365 through the
following programs to enhance your ability to provide
coordinated care for your patients and promote
integration of care among Pennsylvania and New Jersey
Medicare Advantage HMO members and their families,
physicians, and community resources:
- Condition management.
Condition management is available to eligible members for common chronic
conditions such as asthma, diabetes, COPD, and
hypertension.
- Case management. Case management provides
support to members who are experiencing complex
health issues or challenges in meeting their health
care goals.
