Health Coaches are available through the following programs to enhance your ability to provide coordinated care for your patients and promote integration of care among Pennsylvania members and their families, physicians, and community resources:

• Condition management. Condition management is available 24/7/365 to eligible Pennsylvania members for common chronic conditions such as asthma, diabetes, COPD, and hypertension.
• Case management. Through a Health Coach, case management provides support to Pennsylvania members who are experiencing complex health issues or challenges in meeting their health care goals.
