The Office Administration/Patient Education Resources Order Form
(formerly known as the Provider Supply Line form) is an online tool available
to providers that can be used to request or replenish educational resources,
such as health and wellness materials and behavioral health guides. These
resources were developed to support the care you provide to AmeriHealth
members. You can submit requests using the online order form for AmeriHealth
New Jersey or AmeriHealth Pennsylvania.
In order to properly fulfill your request, you will need to provide the
following office information:
- National Provider Identifier (NPI)
- mailing address
- office phone number
Orders are usually processed and shipped within 48 hours and should arrive
within 5 – 7 business days.
