Coverage criteria in the policy for Mohs' micrographic surgery (MMS) were revised to include and define the specific anatomical locations and lesion characteristics and to include the appropriate medically necessary diagnosis codes. Claims for MMS must include one of the diagnosis codes listed in Attachment A of the policy.

For more information about MMS, please refer to Medical Policy #11.08.23i: Mohs' Micrographic Surgery, which was posted as a Notification on December 27, 2017, and will go into effect March 27, 2018.
