Credentialing out-of-state providers
To facilitate the number of professional providers who are available to provide COVID-19 care to our members, AmeriHealth is allowing out-of-state professional providers with a temporary license from the State of New Jersey, the State of Delaware, or the Pennsylvania Department of State (Department) to join an existing group or form a new group in our network during the public health emergency.
Providers seeking to be credentialed in the AmeriHealth networks with a temporary license must first apply for a temporary license from the State of New Jersey, the State of Delaware, or the Department. Once the application is approved by the State of New Jersey, the State of Delaware, or the Department, the provider must complete a Practitioner Participation Form as follows:
Validation for new credentials will include:
- an active, unrestrictive license to practice in the provider’s home state, and
- current malpractice insurance coverage that meets current state guidelines.
On the Practitioner Participation Form, be sure to select the box at the top:

Once the Practitioner Participation Form is received and the validation process is completed, we will send an attestation via DocuSign for the authorized representative of the provider group that hosts the out-of-state provider to complete. For new groups, execution of a participating provider agreement is required and will be sent along with the attestation.
We will work expeditiously to process the request once the completed attestation and participating provider agreement, if applicable, is received.
The temporary provisional participation status will remain in effect throughout the duration of the public health emergency.
