The professional and facility payer ID grids contain valuable information to assist you in claims submission, including prefixes, payer information, and claims mailing addresses by product.

You can download the most current versions from our websites:

• AmeriHealth New Jersey. Go to Provider Forms under Claims Requirements and Communications and select either Facility Payer ID Provider Number. Reference or Professional Payer ID Provider Number Reference.
• AmeriHealth Pennsylvania. Go to Electronic Data Interchange (EDI) under EDI Resources and select either Facility Payer ID or Professional Payer ID.
