Steps to become a credentialed provider with Independence

​Independence requires professional physicians and specialists to complete the credentialing application process before providing care to Independence members.*

To avoid delays in the the credentialing application process, be sure to complete each step of the submission process.

If you have a primary practice address in Pennsylvania or Delaware and would like to join our network(s), you must apply for credentialing by completing both of the following:

If your practice contracts with multiple health plans, the CAQH ProView application minimizes the administrative work needed to fill out multiple, redundant, and time-consuming forms.

*Behavioral health providers must be credentialed by and contracted with Magellan Healthcare, Inc., an independent company.
