Eligible practices can earn financial incentives for providing quality health care and effectively managing the care of their HMO and P​PO members, as w​ell as BlueCard® PPO members. Population Health Specia​lists (PHS) are assigned to each practice and can assist you in improving QIPS performance and overall patient health outcomes.
