Effective August 24, 2026, the Independence Blue Cross (IBX) and Independence Administrators Dosage and Frequency Program will expand to include five new drugs.

The following drugs will be added to this program:
- Darzalex® / Darzalex Faspro®
- Imfinzi®
- Keytruda® / Keytruda QLEX™
- Opdivo® / Opdivo Qvantig®
- Tecentriq® / Tecentriq Hybreza™
Review dosage and frequency requirements
During precertification, IBX and Independence Administrators verify that drug regimens meet the U.S. Food and Drug Administration (FDA) or company-recognized compendia. For specific requirements on each drug's dosage and frequency guidelines, refer to the drug's medical policy in the Medical and Claim Payment Policy Portal.
