Independence Blue Cross (IBX) is making pharmacy dispensing limit changes effective April 1, 2026, to enhance patient safety, reduce waste, and help manage costs. Providers will not need to change how they prescribe medications. The pharmacy dispensing limit changes will include three key components: Split fill for oral oncology medications (for certain IBX members), oversupply prevention, and dispensing optimization.
Split fill for oral oncology medications
For IBX members in a fully insured benefit plan (not an employer self-funded plan) who are starting therapy, select oral oncology medications will be dispensed on a 15-day split-fill schedule for the first 90 days of treatment. This allows for closer monitoring, quicker therapy adjustments, and reduction of medication waste.
Providers should continue to write prescriptions as usual.
Split-fill dispensing will be handled by the pharmacy.
Members will be informed of the new split-fill schedule when their medications are filled.
Oversupply prevention
Oversupply prevention will help ensure members get refills at the right time (not too early, not too late) with an appropriate refill grace period. IBX will review a member's refill history from the past year to see how much medication they already have on hand. If it's too early for a refill, the request will be denied, and the member will be able to refill on the next available date. This process is fully claims-based and does not require any provider action.
Dispensing optimization: Savings through packaging efficiency
Dispensing optimization helps reduce waste and costs through efficient packaging (right package sizes or formats, streamlined processes, etc.) for select medications resulting in meaningful savings without affecting clinical outcomes. This process does not require any provider action.
