ACP services for Commercial and Medicare Advantage members are covered and eligible for reimbursement under our claim payment policies:
When billing for ACP, use CPT® codes 99497 and 99498. To enhance documentation, providers are encouraged to report ACP in the medical record using CPT II tracking codes 1124F and 1125F, as appropriate.
