Reviewing your provider demographic information in the Independence provider directory

If information in the Independence provider directory is incorrect, it has a negative impact on our members. Therefore, it is critical that you regularly review your demographic information in the provider directory to ensure that all of the information is accurate.

In compliance with additional guidance issued by the Centers for Medicare & Medicaid Services (CMS), Independence is working to ensure the accuracy of our provider directory. To support the success of this important initiative, you may have recently received a 2018 Provider Demographic Profile for your review. We ask that you complete and sign your 2018 Provider Demographic Profile by the due date stated on your profile. If no updates are required, you are still required to return the signed profile as confirmation that your information remains accurate.

Impact

If we do not receive your signed profile by the due date stated on your 2018 Provider Demographic Profile, your information will be suppressed from the provider directory. Once your signed profile is received, your information will be updated to display in the directory.

Completing your profile

As you review your profile, we ask that you complete all fields to the best of your ability. Certain fields are included to satisfy CMS requirements. These required fields have been identified below and are marked with an asterisk (*) on the profile.

The following questions MUST be answered, or the profile will NOT be considered a complete response.

For the practice location:

  • Is this office location accepting new patients?
  • Are patients able to schedule appointments at this office location using the phone number listed above?

For the practitioner:

  • Specialty practiced at this location?
  • Sees patients at this location?
  • Accepting new patients?
  • Accepts Medicare Advantage?

In addition to answering the questions for the practitioners, you must provide effective dates for adding and deleting practitioners from the practice.

Once your review and any updates are complete, please sign and date the last page and then fax the entire profile to Independence at 215-238-7074. You must return a signed and dated demographic profile even if you don't have any updates to your practice information.

Review your provider information regularly

Provider offices should review their demographic information published in the provider directory on a quarterly basis. You can view your current listing online using the Find a Doctor tool (i.e., online provider directory).
