us to continue to provide you with important information on billing, claims, 
changes or additions to policies, and announcements of administrative processes.
Professional providers
Please submit changes to your information using the 
Provider Change Form. You can also 
submit changes to your practice information via the NaviNet
® web 
portal, or contact your Network Coordinator to report changes.

You are required to submit any changes to your information in writing. This
