In February, IBC introduced a new health risk assessment, the PraPlus? Health Risk Screening Tool for Seniors for Medicare Advantage HMO and PPO members. This assessment enables IBC to identify the specific health care needs of our members who may benefit from our case management programs. Members are encouraged to complete the assessment and return it to IBC. Primary care physicians of members who return the assessment will receive a letter, which includes an overall summary and individual scores for the members risk of repeat hospital admissions, frailty, and geriatric depression. We encourage physicians to use this information to address their patient?s medical conditions and needs and consider placing the Health Profile and Care Summary in the patient?s medical record to discuss at their next visit.
