to assist you in claims submission, including alpha prefixes, payer
information, and claims mailing addresses by product.
The grids have recently been updated to reflect additions and deletions of
alpha prefixes for Account-specific National PPO, Third-party administrators,
and additional products. Please be sure to download the most current versions,
which are available on our Electronic Data
