As of August 10, 2018, the drug OnpattroTM
(patisiran) requires precertification for all Independence commercial and
Medicare Advantage members.
We are in the process of creating new medical policies for Onpattro. In the
interim, requests for this drug will be evaluated in accordance with the
manufacturer's prescribing information and the U.S. Food and Drug
Administration labeling.
