Impacted cerumen removal is defined as a procedure to extract hardened or 
accumulated cerumen from the external auditory canal by mechanical means, such 
as irrigation or debridement.
 
As a reminder, providers who perform services for the removal of impacted 
cerumen are required to apply Modifier 25 to the evaluation and management 
(E&M) service to identify a significant, separately identifiable E&M service by 
the same physician or other qualified health care professional on the same day 
as the procedure or other services.
