As previously communicated, as of December 1, 2016, providers who submit
paper claims must send them to a new address. A single P.O. Box is now
being used for Keystone Health Plan East, Personal Choice®,
National PPO, and Independence BlueCard® claims:
- Claims Receipt Center
- P.O. Box 211184
- Eagan, MN 55121
If you have not done so already, we ask that you update any internal claims
submission procedures. Use the new address to avoid any potential delays in
claims processing. If you have any questions, please contact Customer Service
at 1-800-ASK-BLUE.
Note: The professional and facility payer ID grids were
updated in January 2017 to reflect this change in address.
