As previously communicated, Keystone 65 Basic HMO has been added to
Independence's lineup of Medicare Advantage plans for 2018. Keystone 65 Basic
HMO is part of the Keystone 65 network of 87,000 physicians and 100 hospitals
and includes Part D coverage, with a 5-tier tailored formulary.
The plan, which boasts a $0 monthly premium, has a medical deductible of
$475. The deductible applies to the following services:
- durable medical equipment
- inpatient hospital care
- inpatient mental health care
- outpatient hospital services
- outpatient observation stays
- outpatient surgery
- prosthetic devices and related supplies
In addition to Keystone 65 Basic HMO, Independence will continue to offer
Keystone 65 Focus Rx HMO, Keystone 65 Select HMO, Keystone 65 Preferred HMO,
and Personal Choice 65
SM PPO.
