New QIO program contractor

As a result of a recent restructuring of the Quality Improvement Organization (QIO) program by the Centers for Medicare & Medicaid Services (CMS), effective August 1, 2014, the existing QIO contractor has been replaced. Therefore, providers will need to include different contact information when completing the Notice of Medicare Non-Coverage form (Form CMS 10123) or the Important Message from Medicare form (Form CMS-R-193).
