Call the Provider Support Line at []1-866-866-4694[] to refer a member to a Health Coach if the member has any of the following conditions: asthma diabetes chronic obstructive pulmonary disease (COPD) coronary heart disease (CHD) heart failure Health Coaches also provide decision support for numerous health-related issues, including back pain, fall prevention, depression, cardiometabolic risk, weight loss surgery, breast or prostate cancer, and chronic pain.
