Independence health rewards is a program where Independence Medicare Advantage members can earn rewards for taking care of their health. Members are advised of the program through direct and digital mail, which lists their eligible activities, ways to redeem for gift cards, and available reward amounts. Signing up for Independence health rewards is easy! Members can activate their account by visiting the health rewards websiteor by calling 1-888-286-1253.

How it works

When an Independence Medicare Advantage member completes a recommended health care activity, he or she can confirm completion by calling the Independence health rewards contact center, by logging into the online portal, or by submitting the required materials by mail. The member will be issued a gift card of their choice, and it will arrive at their address on file within 2 – 4 weeks by mail.* Members may also opt for an e-gift card.

Eligible activities

All Independence Medicare Advantage members are eligible for the activities listed below based on their health status. Members may complete activities from January through December 2019 and attest through any channel (i.e., mail, call center, electronically) beginning April 2019.
