New fax form for procedure code update requests

Effective immediately, all requests to add, delete, or otherwise change a procedure code on an existing authorization should be submitted via fax using the Request to Update Procedure Code(s) on an Existing Authorization fax form. >> []Request to Update Procedure Code(s) on an Existing Authorization[] Note: All requests from NaviNet enabled providers made by phone to add, delete, or change procedure codes will not be accepted after Monday, April 22, 2013.
