As previously communicated, effective November 1, 2010, the Network 
Credentialing Support Services Hotline, 215-241-4120, previously designated for requesting 
your contract and billing registration forms, was replaced with an email 
address, Application_Requests@ibx.com. 
Please note that the hotline has been disconnected.
Providers interested in participating in our network are responsible for 
notifying IBC upon completion of their Council for Affordable Quality 
Healthcare (CAQH) credentialing form. Once the form is completed, providers 
should notify the Network Credentialing Support Services department using the 
email address indicated above. Please include the following information in your 
email:
- 
practitioner's name 
- 
practitioner's mailing address 
- 
practitioner's office address 
- 
practitioner's specialty
- 
practitioner's CAQH ID number
- 
contact person's name, telephone number, and email address.
