Independence is introducing a new pharmacy program for certain Medicare
Advantage HMO or PPO members.
We have contracted with ExactCare?
Pharmacy to offer unique medication packaging solutions for Medicare
Advantage members who are currently taking six or more maintenance medications.
ExactCare Pharmacy specializes in managing medication refills for members with
multiple disease states and complex medication regimens. In addition to
promoting medication adherence for oral medications, ExactCare Pharmacy is also
set up to dispense and deliver unit-of-use products (e.g., inhalers, insulin)
and over-the-counter products.
Eligible members will receive a letter
from Independence advising them of their eligibility for this new program, and
to expect an outreach call from ExactCare, further explaining the program.
Upon enrollment, a pharmacist from ExactCare will contact the member and begin
the following:
- conduct medication reconciliation;
- coordinate monthly fills of prescriptions;
- offer to replace traditional pharmacy vials with unique multi-dose
adherence packaging.
In addition, the pharmacist may contact providers to obtain clarification
regarding the member?s medication regimen.
ExactCare?s packaging
synchronizes all medication refill schedules and delivers a single monthly
package directly to the member?s home.
We ask providers whose patients
are interested in this program to support those members interested in this new
service and cooperate with ExactCare?s outreach efforts.
