QPM score program for measurement year 2013 phase two responses due February 3

Eligible primary care physicians were sent the Quality Performance Measure (QPM) score program phase two mailing in early January. We want to remind you that response forms must be returned to IBC by February 3, 2014. If you have not yet sent your response, please complete each response form by following the instructions in the mailing based on the information from your patients' medical records. You must sign and date the attestation as well as provide proper documentation from your patients' medical records. Once complete, use the pre-addressed UPS label to return the response forms and supporting documentation to us. Make sure to retain a copy of the completed forms and signed attestation for your records. Any information submitted after February 3 will not be included in the final measurement year 2013 calculations. If you are a primary care practice and your office did not receive this mailing, please contact your Network Coordinator as soon as possible.
