Effective October 1, 2014, Nexium® will be 
removed from the Select Drug Program® Formulary. This change 
will move the drug from Tier 2 (formulary level of cost-sharing) to Tier 3 
(non-formulary level of cost-sharing). Additionally, Nexium® (40 
mg capsules and packets for suspension) will now require prior 
authorization.
 
Generic proton pump inhibitors (PPI), such as lansoprazole, omeprazole, 
pantoprazole, and rabeprazole, are available on Tier 1 (the lowest level of 
cost-sharing) with no requirement for prior authorization. 
Nexium® is also available over-the-counter (OTC), along with 
other PPIs such as Prevacid® and Prilosec®.
 
Please discuss the generic PPI alternatives and the proper use of OTC 
products with your patients to determine the most appropriate plan of action. 
As a reminder, using the lowest dose possible for the shortest duration will 
help to minimize potential long-term adverse effects from these agents. Also, 
patients can help to alleviate symptoms by incorporating lifestyle 
modifications including, but not limited to: 
- weight loss
- smoking cessation
- not overeating
- avoiding acidic foods
