programs to enhance your ability to provide coordinated
care for your patients and promote integration of care
among members and their families, physicians, and
community resources:
- Condition management. Condition management is
available 24/7/365 to eligible members for common
chronic conditions such as asthma, diabetes, COPD,
and hypertension.
- Case management. Through a Health Coach, case
management provides support to members who are
experiencing complex health issues or challenges in
meeting their health care goals.
