In an effort to assist you in staying informed of the care your patients are receiving, a new hospital census report for your HMO patients is now available daily for all primary care practices. This report contains information such as hospital name, patient name, diagnosis, and date of admission. You can have this report faxed to your practice daily, Monday through Friday. Each patient will remain on the list until a discharge date is entered into our system. If you are interested in receiving this report, please email your practice name and contact information to IBC at pcmh@ibx.com. You will be contacted by an IBC representative to enroll and begin receiving this new report.
