​The Federal Employee Program (FEP) Director's office of the Blue Cross and Blue Shield Association has informed Independence that the FEP Consent Form for Case Management has been updated to include an additional line for the member's phone number. The updated form is available on our website.

Going forward, please use the updated form when a member agrees to participate in the Case Management Program administered by FEP.
