​​​Changes to medical policy on orthopedic footwear

​Effective June 6, 2022, the Independence medical policy on orthopedic footwear is being updated for all Medicare Advantage members. The following changes are being made in accordance with the Centers for Medicare & Medicaid Services:

  • face-to-face requirement language is being added for HCPCS code L3330: Lift, elevation, metal extension (skate)
  • documentation requirements are being added
